Rhinoplasty Techniques

Two techniques may be used in rhinoplasty:
The closed technique
is preferred because the incisions are placed inside the nose, resulting in minimal scarring.
The open technique
allows Dr. Metzinger greater access to the cartilage and bone within the nose. This method requires an incision across the columella or the underside of the cartilage between the two nostrils, leaving a nearly invisible scar.
Dr. Metzinger determines which type and technique will yield the best results during the initial consultation.

Before Rhinoplasty

Consultation
During the consultation, Dr. Metzinger evaluates the patient’s nasal structure, facial features, and skin thickness while discussing the desired outcome and setting realistic expectations. Medical history, including previous surgeries, medications, and breathing difficulties, is thoroughly reviewed. Dr. Metzinger typically takes photographs from multiple angles and may use computer imaging to visualize potential results.
Preparation
In the weeks before surgery, patients must avoid certain medications and supplements, such as aspirin and vitamin E, that can increase bleeding risk. Smoking cessation is crucial for at least a month, as nicotine may impair healing. Laboratory and physical examinations are often required to ensure the individual is healthy enough for surgery. Patients should also arrange post-operative care, including transportation home after the procedure and assistance during the initial recovery period.

Rhinoplasty Procedure

Anesthesia
Most rhinoplasties are performed under general anesthesia, though some minor corrections can be done under local anesthesia with sedation. Our anesthesiologist provides medications through an IV line, and the patient’s vital signs are continuously monitored throughout the cosmetic procedure.

There are two main incision techniques:

  • In open rhinoplasty, an external cut is made across the columella (tissue between nostrils).

  • In closed rhinoplasty, all incisions are created inside the nostrils.

Dr. Metzinger chooses the approach based on the specific corrections needed.

Once open incisions are made, the skin is carefully separated from the underlying bone and cartilage framework. This creates access to the structural elements that need modification while preserving the blood supply to the skin envelope.
If needed, the nasal bones are carefully fractured in a controlled manner (osteotomy) to narrow the nose or correct asymmetry. Dr. Metzinger uses specialized instruments to reset the bones into the desired position.
Our facial plastic surgeon modifies the cartilage framework to achieve the desired shape. This may involve trimming, suturing, or reinforcing the structures. Sometimes, cartilage grafts from the septum, ear, or rib are used for additional support or improved shape.
The nasal tip can be refined through trimming, using suture techniques to define the tip, or adding grafts for better projection and definition.
If a deviated septum is present, it is straightened through an additional procedure called septoplasty. Dr. Metzinger removes or reshapes portions of the cartilage and bone to improve breathing while maintaining structural support.First Few Weeks
If nostril size reduction is desired, small wedges of tissue can be removed at the base of the nostrils. These incisions are carefully placed in the natural creases to minimize visible scarring.
Dr. Metzinger makes final adjustments to ensure symmetry and proper proportion. The skin is redraped over the modified framework, and internal tissues are carefully positioned.
Incisions are meticulously closed with fine sutures. Internal splints or soft silicone tubes might be placed inside the nose to support the septum. An external splint is applied to protect the nose and help maintain its new shape during initial healing.
The surgical site is cleaned, and dressings are applied. The patient is monitored in recovery until the effects of anesthesia wear off. Specific post-operative instructions are provided for optimal healing.

Week 1

Breathing through the nose is difficult, and the head must remain elevated during sleep. Pain and discomfort are managed with prescribed medications. Cold compresses can help reduce swelling.
The nasal splint and stitches are typically removed during this period. Bruising begins to fade significantly, though swelling remains noticeable. Breathing through the nose improves gradually. Patients can usually return to work or school but should continue avoiding strenuous activities. Some numbness in the nasal area is normal, and the nose remains sensitive to touch.

During this period, most swelling subsides, revealing more defined nasal contours. Numbness and sensitivity gradually disappear.
